Beyond Fentanyl: The New Synthetic Opioid Threat Facing Employers

The Evolving Synthetic Opioid Crisis

The opioid crisis in the United States continues to evolve. While fentanyl remains the synthetic opioid most commonly associated with overdose deaths, a new generation of highly potent synthetic opioids is emerging in the illicit drug supply. These substances, including nitazenes, carfentanil, and other novel synthetic opioids, present significant challenges for public health officials, healthcare providers, law enforcement agencies, and employers.

According to the Centers for Disease Control and Prevention (CDC), synthetic opioids are involved in the majority of opioid overdose deaths in the United States. The rapid rise of fentanyl transformed the opioid epidemic over the last decade, but experts increasingly warn that newer compounds may further complicate prevention, detection, and treatment efforts.

Nitazenes: A New Generation of Highly Potent Synthetic Opioids

Nitazenes are among the most concerning emerging synthetic opioids. Originally developed in the 1950s as potential pain medications, nitazenes were never approved for medical use. However, they have recently appeared in the illicit drug supply and have been linked to overdose outbreaks across multiple states. Examples include isotonitazene, metonitazene, protonitazene, and etonitazene.

Some nitazenes are believed to be as potent as or even more potent than fentanyl, creating substantial overdose risks even in extremely small quantities.

One of the greatest challenges associated with nitazenes is that users often do not know they are consuming them. These substances have been detected in counterfeit prescription pills, heroin, cocaine, and fentanyl mixtures. Because nitazenes are frequently mixed into other drugs, individuals may unknowingly expose themselves to a highly potent opioid. This unpredictability increases the likelihood of overdose and complicates emergency medical response.

Carfentanil Remains a Serious Threat

Carfentanil presents another significant concern. Originally developed as a tranquilizer for large animals such as elephants, carfentanil is estimated to be approximately 100 times more potent than fentanyl and roughly 10,000 times more potent than morphine. Even minute amounts can be fatal.

Although carfentanil receives less public attention than fentanyl, its appearance in the illicit drug supply continues to concern public health and law enforcement agencies.

Xylazine and Medetomidine Complicate Overdose Response

Complicating matters further is the increasing presence of non-opioid substances such as xylazine and medetomidine. These veterinary sedatives are often found in combination with fentanyl and other synthetic opioids. While naloxone remains the standard treatment for opioid overdose, it does not reverse the effects of xylazine or medetomidine. As a result, overdose responses have become increasingly complex, requiring rapid medical intervention and supportive care.

Why Emerging Synthetic Opioids Matter to Employers 

The emergence of these substances has important implications for employers. Many organizations continue to view opioid misuse primarily through the lens of prescription medications or heroin use. However, today’s drug environment is fundamentally different. Employees may unknowingly consume counterfeit pills that contain fentanyl, nitazenes, or other dangerous substances. These pills are usually produced in substandard conditions, labeled incorrectly, and may include dangerous, unapproved substances. There are no quality control mechanisms in the illicit labs producing counterfeit pills to ensure dosing is not lethal. Online sales via internet marketplaces and social media are the major sources for obtaining counterfeit pills. This means that overdose risk is no longer limited to individuals with known substance use disorders.

Employers should recognize that synthetic opioids are both a public health issue and a workplace safety issue. Industries with higher rates of workplace injuries, including construction, manufacturing, transportation, warehousing, healthcare, and public safety, may face elevated risks. Impairment caused by opioids or other substances can increase the likelihood of workplace accidents, injuries, absenteeism, reduced productivity, and higher healthcare costs.

Can Workplace Drug Testing Detect Synthetic Opioids?

One common misconception is that workplace drug testing can reliably detect all dangerous substances. In reality, many standard drug testing panels do not routinely detect fentanyl, nitazenes, or other emerging synthetic opioids. Traditional five-panel drug tests typically screen for marijuana, cocaine, amphetamines, opiates, and PCP. Synthetic opioids like fentanyl require specific lab methods and lower cutoff levels than standard opioid tests. Specialized testing may be required to identify fentanyl analogues or nitazenes, and new compounds often emerge faster than laboratories can develop and validate testing methods. Employers should confirm with their lab exactly which analytes are included, as “opioid” panels can vary. 

Why Supervisor Training and Impairment Recognition Matter

Because of these limitations, experts increasingly recommend that employers focus on impairment and fitness-for-duty rather than relying exclusively on drug testing. Supervisors should be trained to recognize signs of impairment, including confusion, slowed reaction times, excessive drowsiness, poor coordination, and unsafe behaviors. At the same time, organizations should avoid asking managers to diagnose substance use disorders or identify specific drugs. The goal is to ensure workplace safety, not to conduct medical evaluations.

Preparing Your Workplace for an Opioid Overdose Emergency 

Employers should also consider incorporating overdose response procedures into workplace emergency plans. Many organizations now stock naloxone and provide training to designated staff members. Similar to automated external defibrillators (AEDs), naloxone can serve as a life-saving emergency intervention when administered promptly. Training employees to recognize signs of overdose—such as unresponsiveness, slowed breathing, pinpoint pupils, and blue or gray skin discoloration—can significantly improve emergency outcomes.

What Employers Should Know About Kratom

Another substance increasingly discussed in workplace and public health settings is kratom. Unlike fentanyl or nitazenes, kratom is not a synthetic opioid. It is a plant native to Southeast Asia that contains compounds capable of interacting with opioid receptors. At lower doses, kratom may produce stimulant-like effects such as increased energy and alertness. At higher doses, it can cause sedation, dizziness, slowed reaction times, and impaired judgment.

Kratom presents unique challenges because it is not typically included in routine workplace drug testing programs. While some individuals report using kratom for pain management or to help reduce opioid use, concerns remain regarding product quality, contamination, inconsistent potency, and potential dependence. From an employer perspective, the primary concern is not whether kratom is legal or illegal, but whether it impairs an employee’s ability to perform work safely.

Building Workplace Policies for an Evolving Drug Landscape 

As synthetic opioids continue to evolve, workplace policies must evolve as well. Rather than attempting to list every emerging drug, employers should develop broad impairment-based policies that address any substance capable of affecting safe job performance. Policies should include fitness-for-duty expectations, reasonable suspicion procedures, employee assistance resources, overdose response protocols, and periodic reviews of emerging drug trends.

The future of the opioid crisis will likely involve increasingly complex drug mixtures, new synthetic analogues, and continued challenges in detection and treatment. Addressing these risks will require collaboration among public health agencies, healthcare providers, researchers, law enforcement, and employers. By focusing on education, safety, preparedness, and support, organizations can play an important role in protecting both their employees and their communities.

Ultimately, the conversation is no longer simply about fentanyl. It is about an evolving and increasingly unpredictable drug landscape that demands awareness, flexibility, and a proactive approach to public health and workplace safety.
